Major Duties:
- Cultivate positive relationships with the individual served and others who support them
- Support individuals to learn and utilize skills for daily living
- Perform personal care tasks
- Ensure individual rights are respected
- Complete documentation for all services delivered
- Ensure all individuals are treated with dignity and respect
- Understand and take measures to recognize and prevent abuse, neglect, and exploitation
Job Responsibilities:
- Provide direct care to assigned individuals
- Perform any necessary self-care tasks, including but not limited to basic personal hygiene, grooming, feeding, ambulation tasks, medical monitoring, and monitoring for health and safety.
- Ensure the individual's environment is safe; assist with maintaining it, if necessary
- Remind and/or assist individuals with self-administration of medications (after Core A/B training is successfully completed)
- Provide support for community activities and engagement
- Assist with and track all necessary PCISP and PBSP goals and strategies
- Encourage individuals to move toward his/her wants and needs
- Assist individuals to plan meals, shop for groceries and other items, prepare/serve meals, feed
- Help individuals with household cleaning and simple maintenance tasks
- Assist with toileting, such could include reminders, verbal prompting, and/or physical assistance
- Complete documentation for all delivered services within the identified timeframe
- Report all incidents immediately with an internal report completed within 12 hours of the incident
- REPORT ALLEGED, SUSPECTED, OR ACTUAL ABUSE, NEGLECT, OR EXPLOITATION IMMEDIATELY
- Attend interdisciplinary team meetings as requested
- Other duties as assigned
Minimum Qualifications:
Education: High school diploma, GED, or equivalent
Experience/Requirements: One year of paid or volunteer experience required; must be 18 years old; must possess a valid, unrestricted driver's license; clean criminal history.
Skills: Able to work independently and with others; competent oral and written communication skills; patience; compassion; adaptability; reliability; ability to work in a home-like setting with one or more individuals; ability to complete physical aspects of the job including bending, stooping, lifting, pushing, pulling, reaching, and walking for periods.
Working Conditions: This position may function within an individual's home and/or community; travel to/from the individual's home required daily; must be able to handle physical aspects of the job, including bending, stooping, lifting, pushing, pulling, reaching, and walking for periods; use of web-based technology, cell phone, and computer required.
Working Schedule: Hours are based on needs of the individual served and required trainings. This may include weekdays, weekends, holidays, and days with inclement weather.
Community Support Specialist (CSS)
Job Description
Main Objective: Provide person-centered direct support services to individuals with disabilities in their community to promote independence, integration, and natural supports
Major Duties:
1. Cultivate positive relationships with the individual served and others who support them
2. Support individuals to learn and utilize skills in their community
3. Perform personal care tasks
4. Ensure individual rights are respected
5. Complete documentation for all services delivered
6. Ensure all individuals are treated with dignity and respect
7. Understand and take measures to recognize and prevent abuse, neglect, and exploitation
8. Advocate and educate community members on how to best support, as applicable
Job Responsibilities:
1. Provide direct care to assigned individuals in the community
2. Perform any necessary self-care tasks, including but not limited to basic personal hygiene, grooming, feeding, ambulation tasks, medical monitoring, and monitoring for health and safety.
3. Remind and/or assist individuals with self-administration of medications (after Core A/B training is successfully completed)

11. Complete documentation for all delivered services within the identified timeframe

Behavior Consultant - (Level 2 Clinician)
Job Description
Main Objective: Provide behavior supports to individuals that exhibit behavioral, social, or emotional challenges which inhibit their ability to achieve self-determination, interdependence, productivity, integration and inclusion in all facets of community life
Major Duties:
1. Provide behavior supports to all assigned individual in a manner that is individualized, personcentered, and consistent with the PCISP created by the individual’s team.
2. Ensure all documentation meets quality and content standards outlined by 460 IAC, most recent DDRS HCBS Waiver Manual, DDRS and BDDS directives, and company policy.
Job Responsibilities:
1. Consult and collaborate with the individual, IST, family, formal and informal supports, and other important people to the individual for completion of the Functional Behavior Assessment and Positive Behavior Support Plan.
2. Complete Functional Behavioral Assessment, ensuring all required components are present.
3. Develop comprehensive positive behavioral support plan and subsequent revisions, ensuring all required components are present.
4. Ensure proactive and least restrictive interventions are utilized first. If proactive and least restrictive interventions are not effective, based on current evidence and documentation, consider more restrictive measures.
5. Ensure all restrictive measures are approved by the individual/legal guardian/IST/HSPP/HRC.
6. Complete contact note for every billed activity completed on behalf or with the individual. Ensure only reimbursable activities, with contact note, are submitted for reimbursement.
7. Support the individual with learning new, positive behaviors as outlined in the PBSP.
8. Train staff, family member, housemates, or other individual on the implementation of the PBSP.
9. Complete quarterly progress reports and submit to HSPP for review, then to IST and state portal.
10. Submit FBA to individual, IST, and HSSP for review and approval.
11. Submit PBSP to individual, IST, HSSP, and HRC (if restrictions present) for review and approval.
12. Allow new behavior consultants with the company to shadow, with the individual’s approval.
13. Must participate in one hour of supervision per month with Level 1 Clinician
14. Complete 10 hours of approved CEUs related to the specific position each year.
Minimum Qualifications:
Knowledge: Master’s degree in: clinical psychology, counseling psychology, school psychology, or another applied health service area of psychology; special education; social work; or counseling; or be a licensed marriage and family therapist licensed under IC 25-23; or be a licensed clinical social worker under IC 25-23; or be a licensed mental health counselor under IC 25-23; or have a master’s degree in a human services field and be able to demonstrate to the BDDS behavior management committee that the individual has either coursework in or five (5) years of experience in devising, implementing, and monitoring behavior support plans
Experience: Three years of experience as a behavior consultant preferred; more than two years working with individuals with intellectual and/or developmental disabilities required; previous experience completing reports
Skills: Strong oral and written communication skills; ability to work with a wide range of individuals; ability to resolve conflict effectively; strong oral and written communication skills; effectively listening skills; ability to use (or an ability to quickly learn) a wide variety of technology.
Working Conditions: This position may function within a home office setting or community setting, using a computer (with online fax), phone (with application-based scanner), and copier. Travel required. Schedule is flexible.
